Course overview
Investigate incidents that affect both service delivery and HSE performance. Connect evidence and causal analysis with service recovery, corrective action and effectiveness checks.
Why this course matters
Service recovery and incident learning require different evidence. Correcting an immediate failure does not by itself establish its underlying causes.
Learning objectives
- Explain the scope of service incidents and HSE events.
- Identify the key requirements for initial response and evidence.
- Apply a structured approach to timeline and causal mapping.
- Evaluate evidence relating to human and organisational contributors.
- Develop a practical output for corrective action and service recovery.
- Justify recommendations concerning implementation tracking and ownership.
- Review responsibilities for effectiveness checks and shared learning.
- Prepare an implementation plan for integrated investigation workshop.
Who should attend
cross-functional corporate teams, operational managers, FM/HSE professionals and contractor interfaces

Review criteria are accuracy, use of evidence, recognition of limitations, clear responsibility and feasibility. The facilitator provides feedback; joining instructions confirm attendance requirements and any formal pass criteria before delivery.
Completion information
